The game comes with an insert that contains your unique code. If you're unable to locate it then I'm left to assume you've purchased a used copy in which case it's no longer a GOTY edition as the code has most likely already been redeemed.
Some poor guy who can't afford $10 stole the code because some retailers leave their booklets and stuff in their cases

I bought the game (New on amazon.com) and couldn't find the code also. There's a web-page were you supposedly redeem the code but it's been on maintenance for a month now. Even if you can't find the code there's a good chance it won't matter because i reckon that site where you redeem that code from won't be ack up.
